    Unless the policy has a portability provision (which is rare) then you are out of luck and the coverage is lost. If it is voluntary disability plan offered by a specialty carrier (like AFLAC, Colonial, American Public Life, etc.) that you purchased through your employer, you may be able to take that with you. You will need to check with either HR or know under what circumstances you were paying for that coverage.
